About the role
Provide telephonic, short-term care management for high-risk patients post-discharge. Advocate and coordinate care to improve health outcomes.
What you’ll do
• Engage newly discharged patients telephonically. • Conduct assessments of patient conditions and history. • Develop and evaluate care plans with patients. • Educate patients on chronic conditions and red flags. • Coordinate care with providers and community resources.
What you’ll bring
• Unrestricted Compact RN license in US. • 3-4 years RN care management experience. • Fully bilingual in English and Spanish. • Located in CT, MT, or PT time zones. • Work Monday-Friday 10am-6:30pm or 10:30am-7pm ET.
