Client Navigator

About the role
Support clients to navigate integrated health services and maintain continuity of treatment and care across HIV, TB, malaria, and MNCH services.
What you’ll do
• Support client navigation and linkage to relevant health services. • Follow up with clients on scheduled appointments. • Promote continuity of treatment and document follow-up actions. • Coordinate referrals and confirm client access to services. • Maintain client records and protect confidentiality.
What you’ll bring
• Nursing, Social Work, Psychology, Public Health or CHEW qualification. • At least one year relevant experience or 3-5 years hospital experience. • Ability to work with facility and community teams. • Commitment to quality integrated health services. • Fluent in Hausa and English; resident of application state preferred.
