Client Navigator

About the role
Support clients to navigate integrated health services, maintain treatment continuity, and complete referrals within project scope.
What you’ll do
• Distinguish general from specialized maternal and child peer-support functions. • Work with facility and community teams per approved procedures. • Coordinate and confirm referrals between services and facilities. • Maintain client records and protect confidentiality. • Support client navigation to HIV, TB, malaria and MNCH services.
What you’ll bring
• Commitment to quality integrated health services. • Fluent in Hausa and English; resident of application state preferred. • Collaborate with facility and community teams. • Nursing, Social Work, Psychology, Public Health or CHEW qualification. • 1+ year relevant experience or 3-5 years hospital experience. • Comply with project, government and facility procedures.
