About the role
Support Medicare patients remotely as a healthcare advocate using your LPN/LVN clinical background. Help patients navigate healthcare and social barriers to improve outcomes.
What you’ll do
• Use LPN/LVN clinical skills with proprietary platform. • Build trust through empathy and attentive listening. • Assess patient situations and prioritize care. • Design care plans addressing clinical and social needs.
What you’ll bring
• Current LPN or LVN license in good standing. • Experience with diverse patient backgrounds. • Compassionate and patient-centered. • Organized and able to manage multiple priorities. • Comfortable learning new technical systems. • Must be based in the United States.
