About the role
The PHRN Clinical Coordinator conducts comprehensive reviews of patient care documentation to ensure quality and compliance. They collaborate with clinical staff to improve patient records and support ongoing education and training.
What you’ll do
• Complete and submit accurate MDS assessments on time. • Develop and update individualized care plans. • Ensure documentation aligns with Medicare/Medicaid and CMS regulations. • Collaborate with healthcare team to support quality patient care. • Review patient records for completeness and accuracy.
What you’ll bring
• Current and valid PHRN license. • At least 1 year bedside/clinical experience. • Minimum 1 year experience with MDS. • Experience supporting U.S. Skilled Nursing Facility. • Work authorization for Philippines.
