About the role
Support patients by monitoring alerts, responding to clinical needs, and providing flexible nursing coverage. Ensure continuity of care for chronic condition patients through triage and care management.
What you’ll do
• Monitor patient vitals and lab data via alert board. • Respond to patient clinical questions by phone and text. • Conduct outreach to patients with abnormal readings. • Follow triage protocols for escalation and emergency referrals. • Document patient interactions and support care coordination.
What you’ll bring
• Active RN compact multi-state license. • Associate Degree in Nursing. • 5+ years clinical nursing experience. • Experience with clinical triage and telehealth. • Strong clinical judgment and communication skills.
