About the role
Provide advanced clinical support for high-acuity cardiac patients remotely. Manage care plans, clinical triage, and patient engagement to improve cardiac outcomes.
What you’ll do
• Perform clinical assessments via phone. • Develop and update care plans. • Lead Transitional Care Management efforts. • Provide patient and caregiver education. • Coordinate with providers and community resources.
What you’ll bring
• Active RN license in Texas or compact state. • 2+ years cardiac care or care management experience. • Knowledge of chronic cardiac conditions. • Excellent communication and documentation skills. • Work authorization for United States.
