About the role
The RN Navigator provides clinical navigation and care coordination for high-risk patients across Mosaic’s ambulatory continuum. The role ensures seamless communication, timely follow-up, and effective care transitions for complex patient needs.
What you’ll do
• Coordinate care of complex and high-risk patients. • Support seamless transitions among care providers. • Manage urgent and complex referrals. • Perform high-risk lab and diagnostic follow-up. • Conduct emergency department follow-up and patient outreach.
What you’ll bring
• Associates degree and Oregon RN License required. • Minimum two years RN clinical experience preferred. • BLS/CPR Certification and valid Oregon driver’s license. • Superior nursing process and communication skills. • Work authorization for United States required.
