About the role
Provide after-hours clinical support to hospice patients and families remotely. Assess needs, coordinate care, and deliver compassionate guidance using Telecare protocols.
What you’ll do
• Provide after-hours triage and clinical assessment via phone. • Support symptom management and end-of-life care needs. • Coordinate care with physicians, facilities, and teams. • Dispatch visits and coordinate vendor services. • Document interactions and care plans accurately.
What you’ll bring
• Active RN license without restrictions. • Hospice experience required. • Experience with Home Care Home Base (HCHB). • Strong clinical judgment and critical thinking. • Excellent communication and phone-based care skills.
